Completion of this form signifies your acceptance of this invitation. Failure to submit this form by May 30, 2022 will automatically forfeit your reserved slot to accommodate those in the waitlist. Information provided upon enrollment will not be sold nor distributed to any external parties. This survey will only be utilized for the purposes of the Zero Waste Academy Online organized by Mother Earth Foundation (MEF) and Global Alliance for Incinerator Alternatives (GAIA).
