Completion of this form signifies your acceptance of this invitation. Failure to submit this form by February 22, 2023 will automatically forfeit your reserved slot to accommodate those in the waitlist.

Information provided upon enrollment will not be sold nor distributed to any external parties. This survey will only be utilized for the purposes of the Zero Waste Academy organized by Mother Earth Foundation (MEF) and Global Alliance for Incinerator Alternatives (GAIA).

The completion of this form signifies your commitment as an OFFICIAL PARTICIPANT to the Asia Pacific Cohort of the Zero Waste Academy Implementing Zero Waste to Cities and Communities Track. As an OFFICIAL PARTICIPANT, you are expected to attend and participate in this 8-week course. By signing this statement of attendance, you are also agreeing not to distribute the course materials for commercial purposes, nor claim them as your own. You are committing to upholding GAIA’s and Mother Earth Foundation’s copyrights to all course contents, and full acknowledgment will be required if any materials are to be used for your program implementations and external discussions. Any violation of these policies, such as claiming materials as your own, or misrepresenting the course contents, will result in a forfeiture of your access to the course materials and the opportunity to participate in future cohorts.
